Drop-off begins January 23
In one envelope INCLUDE ALL of the following:
Copies of social security cards or ITIN’s for you and the entire household
Copies of Government-issued photo IDs
W-2 and 1099 forms from every employer
Voided check for direct deposit
Intake Form (download/complete)
Intake Form in Spanish (download/complete)
Bank Form (download/complete)
All forms must be provided to complete tax preparation.
How to drop off your documents:
Please confirm that you have all required documents in ONE envelope. You can bring in your documents during open hours, or pick up an intake packet in the box outside of the office and drop off in the night drop.
